By applying person-centred planning, values-driven frameworks, and a range of tools and approaches, I walk alongside you to identify you strengths and barriers to develop meaningful solutions, and help you move closer to where you wish to be.

Various NDIS Capacity Building line items can be used, depending on your plan and  support needs.

-Assessment Recommendation Therapy or Training - Social Worker - $193.99 per hr.

-Life Transition Planning Incl. Mentoring PeerSupport And Indiv Skill Develop - $80.06 per hr.

-Community Engagement Assistance - $51.20

For community members wishing to access my service I will work with you to ensure fees take into account your personal situation. 

The Support Coordination role is varied and dynamic. Depending on the number of approved hours, I will help you:

- Understand your NDIS plan 

- Connect you with a Plan Manager

- Track & plan your funding

- Connect in with suitable NDIS, community, and mainstream service providers

- Set up service agreements and navigate the required admin with new providers

- Document progress and achievements toward your goals and send updates to NDIA

- Submit requests for reviews of your plan 

- Call the NDIA so you don't have to (this will be the one you appreciate the most)

We will work out a plan together based on the number of approved hours. 

Although the NDIS adult age criteria is 9 years old, I only support NDIS participants 18 years and over. 

-Supporting Agency, Plan & Self Managed NDIS Participants. $100.14 per hour.
